Any group within the State of Kansas may borrow these DVD's at no expense to them except for paying return postage. We do not send out our DVD's out of State.
All videos in the library are in a DVD format. Indicate if substitutions can be made if the requested item is not available or list a second choice of date. They may also be picked up and returned in person. Since most items are heavily booked, it is mandatory that they be returned on the date due. You could lose your privilege to use the library, if DVD's are not returned on time. Delay of not returning them results in others not receiving them on time.
When returning DVD's please do not tape the whole box up you only need one piece of tape.
Request items three weeks in advance. Please use item number, date and name of video when ordering. Only written requests will be accepted. Forms are included.
Passing a video onto another borrower is "NOT" permitted.
Return DVD's in the container they arrive in.
Fill out the users report slip enclosed with the DVD's and return it immediately after showing.
Do not schedule any DVD's for showing without checking with the library first.
If there is damage to the video please be sure to mark it on the enclosed user's report slip.
It is the responsibility of the user to put a 'tracer' on the DVD's if it is lost or misplaced in the mail.

Macintosh users must have Acrobat or Reader installed and be viewing the PDF in Acrobat or Reader for the submit button work. Many Mac users have the Preview Application as their default PDF viewer, and it does not support many interactive features such as form submissions. For those that do not use PDF viewers that support form submissions, they will have to manually attach the document to an email message. Click on the link below for more help.
