Instructions for Upgrading an Initial School Psychologist License to a Professional License Based on Internship - Form 19

Step 1 - Register for a Login
Before you can fill-out and submit your application you must register a user name and password. To register, do the following:

  1. Access the *Kansas Licensure Application System website
  2. Click on the "Register" button to display the "KSDE User Registration Form".
  3. Follow the steps to complete the registration process.

Step 2 - Complete Application Form 19
To complete your application, do the following:

  1. Access the authenticated public applications website.
  2. Enter your user name and password, then click the "Login" button. NOTE: If you forget  your password, click the "Forgot Your Password?" link and answer the questions.
  3. Once you have successfully logged in, you will see the "KSDE Web Applications" page. Click on the "License Application" menu option to access Form 19 - Upgrade Initial School Specialist to a Professional Based on Internship.
  4. Fill in the form.
  5. If you want to keep a copy of your application form for your files, choose the print button at the bottom of the page.

Step 3 - Submit the Completed Form
Choose the submit button at the bottom of the page when you are ready to submit. Your application will be sent to the university you designated on the form. When your university completes and submits, the application will be sent to KSDE.

Step 4 - Submit Application Fee to KSDE
As soon as your university submits your completed application to KSDE, you will be notified by email with complete instructions on how to submit the application fee.

IMPORTANT NOTE:
An application is not acceptable without the fee and will not be processed until the fee is received.
