Instructions for applying for a second initial license within five years of issuance of the first initial license

Step 1 - Register for a Login
Before you can fill-out and submit your application you must register a user name and password. To register, do the following:

  1. Access the authenticated public applications website.
  2. Click on the "Register" button to display the "KSDE User Registration Form".
  3. Follow the steps to complete the registration process.

Step 2 - Complete Application Form 20
To complete your application, do the following:

  1. Access the authenticated public applications website.
  2. Enter your user name and password, then click the "Login" button. NOTE: If you forget  your password, click the "Forgot Your Password?" link and answer the questions.
  3. Once you have successfully logged in, you will see the "KSDE Web Applications" page. Click on the "License Application" menu option to access Form 20 - Initial License Renewal.
  4. Fill in the form.
  5. If you want to keep a copy of your application form for your files, choose the print button at the bottom of the page.

Step 3 - Submit the Completed Form
Choose the submit button at the bottom of the page when you are ready to submit. As soon as you submit, you will instantly receive a confirmation message on the computer screen that includes complete instructions on submitting the application fee.

IMPORTANT NOTE:
An application is not acceptable without the fee and will not be processed until the fee is received.
