Steps, if any, that were taken by the school or the district regarding the incident for each offender associated with the incident. Action Taken is further defined by one of the following classifiers: minor punishment, detention, conference, confiscation of contraband, demerit, probation, restitiution, Saturday school, in-school suspension, removal or out-of-school suspension, expulsion, student transfer, referred to local law enforcement, no school response, or other. All suspensions, expulsions, and referrals to local law enforcement are required to be reported.

Drinking alcoholic beverages.

To unlawfully and intentionally damage or attempt to damage any school or personal property by fire or incendiary device.  Firecrackers, fireworks, and trashcan fires would be included in this category if they were contributing factors to a damaging fire.

Causing physical harm to an individual(s). Assault is further defined by one of the following classifiers: physical, sexual, fighting, kidnapping, robbery, murder/homicide, other. All incidents involving violence are required to be reported.

Mutual participation in an incident involving physical violence, where there is no major injury.

Unlawful seizure, transportation, and/or detention of a person against his/her will, or of a minor without the consent of his/her custodial parent(s) or legal guardian. This category includes hostage-taking.

Killing a human being.

Type of assault incident other than those listed above.  Requires description.

Touching or striking of another person against his or her will or intentionally causing bodily harm to an individual.  (Battery)

The taking of, or attempting to take, anything of value that is owned by another person or organization under confrontational circumstances by force or threat of force or violence and/or by putting the victim in fear.

Oral, anal, or vaginal penetration forcibly or against the person’s will or where the victim is incapable of giving consent. Includes rape, fondling, indecent liberties, child molestation, and sodomy.

Any act that disrupts the orderly conduct of a school function. Behavior is further defined by one of the following classifiers: disruptive, obscene, insubordination, failure to serve sentence, or other.

Refusal to serve or willful noncompliance with discipline action(s) taken as a result of a previous discipline incident; particularly related to detentions, Saturday school, probation, and restitution.

Behavior which substantially disrupts the orderly learning environment.  - Does not involve violence, property damage, or obscenity.

Unwillingness to submit to authority, refusal to respond to a reasonable request, or other situations in which a student is disobedient.

Language or actions, written, oral, physical, or electronic, in violation of community or school standards.

Type of behavior incident other than disruptive, obscene, insubordination, or failure to serve sentence.  Requires description.

An administrator discussed the incident with the student and issued a warning regarding the consequences of subsequent offenses.

Selling or purchasing illegal drugs or over-the-counter medication in violation of school policy.

Smoking, snorting, injecting, ingesting, or otherwise using an illegal drug or over-the-counter medication in violation of school policy.

Selling or purchasing marijuana or another cannabinoid.

Smoking, snorting, injecting, ingesting, or otherwise using marijuana or another cannabinoid.

Having marijuana or another cannabinoid in one’s pocket(s), bag(s), car, locker, etc.

Having equipment (e.g. bong) for use in consuming illegal drugs in one's pocket(s), bag(s), car, locker, etc.

Incident is related to the possession, use, solicitation, or sale of illegal drugs or controlled substances in violation of school policies. Drug-Related is further defined by one of the following classifiers: marijuana possession, marijuana use, marijuana solicitation/sale, illicit drug possession, illicit drug use, illicit drug solicitation/sale, drug paraphernalia possession, or drugs-other. All incidents involving drugs are required to be reported.

Those instances in which an impartial hearing officer orders the removal of children with disabilities from their current educational placement to an appropriate alternative educational setting for not more than 45 days based on the hearing officer’s determination that the public agency has demonstrated by substantial evidence that maintaining the child’s current placement is substantially likely to result in injury to the child or others.

The student was expelled from and is no longer enrolled in his or her regular school setting.  Arrangements were made for the provision of educational services.

The student was expelled from and is no longer enrolled in his or her regular school setting.  Total cessation of educational services.

Altering, copying, or imitating of something, without authority or right, with the intent to deceive or defraud by passing the copy or thing altered or imitated as that which is original or genuine; or the selling, buying, or possession of an altered, copied, or imitated thing with the intent to deceive or defraud.

To unlawfully bet or wager money or something else of value; assist, promote, or operate a game of chance for money or some other stake; possess or transmit wagering information; manufacture, sell, purchase, possess, or transport gambling equipment, devices, or goods; or tamper with the outcome of a sporting event or contest to gain a gambling advantage.

Expulsion length was shortened or changed to a suspension by the superintendent under the case-by-case modification provisions of Section 4141(b)(1) of the Gun Free Schools Act. A description of the modification is required.

Indication of whether the student had an IEP for disability as defined in IDEA at the time of the incident.

Use of school computer equipment for inappropriate uses such as viewing inappropriate material, changing school records, etc.

Incidents other than assault, weapons, threat, alcohol-related, drug-related, tobacco-related, arson, behavior, forgery, gambling, theft, sexual offenses-non-violent, trespassing, truancy, vandalism, inappropriate computer use, cumulative or due process determination.  Requires description.

The student was removed from his or her regular classroom and assigned to an in-school suspension program for a specified length of time.

Length of Suspension or Expulsion in school days. All expulsions and suspensions lasting more than one full school day are required to be reported.

Local numeric identifier that is created by school/districts in their SIS system, and which allows the same incident to be edited and re-imported into KAN-DIS without creating a new incident. This ID is required to be used by schools utilizing the import option (for importing discipline incident into KAN-DIS from other software products).

Location where the incident took place. Location Group is further defined by one of the following classifiers: on campus or off campus.

Incident took place off the school grounds. Off Campus is further defined by one of the following classifiers: sponsored activity off campus, sponsored transportation, or other off campus.

Incident took place on school grounds. On Campus is further defined by one of the following classifiers: classroom, hallway/stairwell, cafeteria, gym, restrooms, auditorium, outside, stadium, or other on campus.

An injury that requires professional medical attention.

An injury that does not require professional medical attention.  Medical attention from the school nurse qualifies the injury as minor unless further medical attention is required.

Punishment did not involve removing the student from the classroom setting.

Indication of the probable motivation that caused the offender to act.  Used for Positive Behavior Support (PBS). Motivation is further defined by one of the following classifiers: adult attention, peer attention, gain item or activity, control, avoid adult, avoid peer, avoid activity, avoid situation, self expression, justice or revenge, or other.

Victim received no physical injury.

No actions were taken by the school in response to the incident.  Description required.

Student currently attending the school where the incident occurred.

Student who attended the school where the incident occurred, but who has been expelled or suspended during the current school year.

Student who previously attended and graduated, dropped out of, or transferred out of the school where the incident occurred.

The student was restricted from certain school functions and/or activities.

The incident was reported to law enforcement officials.  Felony Charges were brought against offender.  Description required.

Repeatedly annoying or attacking a student or other personnel which creates an intimidating or hostile educational or work environment related to that individual specifically. (Bullying)

Repeatedly annoying or attacking a student or group of students or other personnel which creates an intimidating or hostile educational or work environment related to unwelcome sexual advances, requests for sexual favors, other physical or verbal conduct or communication of a sexual nature, including gender-based harassment.

Physical, verbal, written, or electronic action which immediately creates fear of harm, without displaying a weapon and without subjecting the victim to actual physical attack.

Any threat (verbal, written, or electronic) by a person to bomb or use other substances or devices for the purpose of exploding, burning, causing damage to a school building or school property, or to harm students or staff.

Victim information is required if the incident being reported involved a victim that sustained a major or minor injury. Victim is furthe defined by one of the following classifiers: current student, expelled/suspended student, former student, student at a different school, teacher, principal/vice-principal, other staff member, parent, unknown, or other.

Student who attended the school where the incident occurred, but who has been expelled or suspended during the current school year.

The nature of the injury sustained by the victim associated with the incident. Victim injury is further defined by one of the following classifiers: no injury, minor injury, or major injury.
