Districts that submitted Techology Plans on 2004 or after will need to complete an Addendum for their E-rate purposes and will use the two following documents as the online submission form for the Addendum is not available. Districts that are currently due for submission for the years covering July 1, 2010 to June 30, 2013 this information will be included in the new Online Submission Portal and does not need to completed separately.

Budget Addendum
When completed, print these documents and store them in your E-rate binder for future use. Do not submit the completed documents to KSDE as individual districts are responsible for keeping their compliance documentation.
