You must put a name in the Custodian of Record and if this is not the Parent or Guardian you must put the Parent/Guardian's name in the Parent/Guardian's information area! Failure to do so means that you have not registered.

To comply with the provisions of KSA 72-53, 100 through 72-53, 102, please provide the information on the form below concerning your school (elementary or secondary).
