If you are downloading an initial out-of-state (PDF Document) or out-of-country (PDF Document) application, you will also need to order a Fingerprint Card.

Fee increase effective September 1, 2011

Effective September 1, 2011, an increase in the licensure application fee will occur. The increase will be on a structured scale by license type calculated as a $3.00 increase per year based on the length of the license

Fingerprint Card and Instructions Order Form

All applicants for an initial Kansas license must submit one fingerprint card for the purpose of a KBI and FBI background clearance report.
One fingerprint card is also required for applicants whose most recent Kansas certificate/license has expired.
